The deductible ceiling

No Medicare drug plan may have a deductible above $615 in 2026. Some plans have no deductible. When a deductible applies, the deductible stage requires payment of out-of-pocket costs until the full deductible is reached. Medicare, Part D costs (opens in a new tab).

Covered-drug out-of-pocket cap

In 2026, out-of-pocket spending on covered Part D drugs reaches the catastrophic coverage stage at $2,100, including certain payments made on a person’s behalf, such as Extra Help. Covered Part D drugs then have no out-of-pocket cost for the rest of the calendar year. Medicare, Part D costs (opens in a new tab).

Separate plan premiums

A premium is a monthly payment for coverage whether covered drugs are obtained or not. Its amount depends on the plan, may be higher based on income, and can include a late enrollment penalty. Medicare, Part D costs (opens in a new tab).
