The levy is a separate charge
Services Australia distinguishes the ordinary Medicare levy from the surcharge. The levy is paid on top of income tax; reductions or exemptions can apply. Services Australia Medicare levy (opens in a new tab).
This guide does not calculate the charge or determine any income, medical or residency exemption.
Hospital cover and the surcharge
The surcharge may apply when income exceeds the relevant threshold and appropriate private hospital insurance is absent. This guide supplies no income threshold, surcharge rate or insurance eligibility decision. Services Australia levy surcharge (opens in a new tab).
A hospital-cover question does not settle the ordinary levy question. Services Australia levy surcharge (opens in a new tab).
The insurance rebate is different
The private health insurance rebate depends on income. It can reduce the premium upfront or be claimed as a tax offset in the annual return. Services Australia insurance rebate (opens in a new tab).
It is a separate arrangement from the levy and surcharge; this explanation does not select a policy. Services Australia insurance rebate (opens in a new tab).
A statement records a period
A Medicare Entitlement Statement records the period in a financial year when the person was not eligible for Medicare. It supports an exemption application in the tax return. Services Australia entitlement statement (opens in a new tab).
The ATO decides whether the levy is payable; receiving a statement does not establish the final tax outcome. Services Australia entitlement statement (opens in a new tab).
A previous statement is not continuing approval
Services Australia states that you must get a new Medicare Entitlement Statement each year you claim an exemption. A prior exemption does not guarantee another exemption. Services Australia annual statement requirement (opens in a new tab).
This guide explains the evidence boundary only: it does not determine eligibility, cover other exemption categories or give application deadlines. Services Australia annual statement requirement (opens in a new tab).
